Service Description

This service includes a 1 hour meeting discussing and elaborating on the intake questionnaire to be completed before consultation. The intake questionnaire will include medical history, diet history, medical symptom questionnaire, health appraisal questionnaire, three day diet diary. Also included is a physical exam and BMI calculation. Once the Initial Consultation is done you can then decide if you would like to move on to a more hands on approach which includes the 6-pack consultation, 3-pack consultation or a follow up consultation.

Cancellation Policy

Classes and class cards are not refundable and must be used within their event date or expiration date. All classes not used within the time frame stated at purchase will be lost. (Pre-sign up for classes to use them all before you lose them). No extensions for class cards & memberships. Class cards and memberships are not transferrable.
